Community Health Connections is a federally qualified health center with four locations in Fitchburg, Gardner and Leominster, MA. Their mission is to provide high quality, affordable healthcare in the community. Community Health Connections joined the CRVFHP in 2021 to help expand access to health services for migratory and seasonal farmworkers, and their families.
Community Health Connections welcomes all patients. Farmworkers and their families who do not have health coverage, or are underinsured are eligible for enrollment in the CRVFHP while they live or work in the Connecticut River Valley.
Farmworkers must be employed in the direct farming of seasonal crops. Agricultural work includes cultivating, harvesting, or preparing seasonal crops for market or storage. This includes anyone who has been employed as a migratory agricultural worker within the last 24 months.
