Our Partner Agencies are the heart of the program. They are health centers in Connecticut and Massachusetts that provide a broad array of high-quality, primary and preventive care services, including screening, diagnosis and management of chronic illnesses. We provide support to ensure the provision of services, including transportation, case management, medical interpreting, and health education to help farmworkers and their families access needed care.
